ENROLLMENT REQUIREMENTS
If you...
OWN OR RENT IN THE LAMPHERE DISTRICT
One of the following:
□ Home Mortgage Statement or Warranty Deed
□ Current Lease or Rental Agreement. Lease must be resubmitted upon expiration.
(Applications are not accepted)
□ Current Property Tax Statement
AND
□ Three (3) pieces of current FIRST CLASS mail with your name and current address.
(P.O. Boxes are not accepted)
LIVING WITH A FRIEND OR RELATIVE
IN THE LAMPHERE DISTRICT
Provide the following:
□ Signed and notarized Affidavit and Consent Agreement (forms available at Lamphere
Administration Center or School office)
□ 3 pieces of FIRST CLASS mail with your name and the homeowners physical address.
(P. O. Boxes are not accepted).
□ 3 pieces of FIRST CLASS mail with the homeowners name plus Home Mortgage papers, current lease* or current Property Tax Statement.
*Lease must include names of all adults residing in household.
ADDITIONAL REQUIREMENTS
For each student, provide the following:
□ Original Birth Certificate, Original Passport , Immigration or Refugee Status Cards
□ Up-to-date Immunization Record
□ Copy of most recent report card, if applicable
□ Vision Screening, Kindergarten Only
