WORK PERMIT PROCEDURE:

1. Students may pick up a blank work permit in the Main Office or Co-op Office. The CA-6 is a pink form and is used for minors who are under 16 years of age. The CA-7 is a yellow form and is used for minors who are 16 and 17 years of age.
2. The minor completes Section I.
3. The minor takes the form to the prospective Employer who completes Section II.
4. The minor takes the form to the Co-op Office in person. Section III of the form will be completed (if the work permit complies with state and federal laws and regulations).
5. Minor returns the completed original form to the employer.
6. Minor may now begin work.
